First Consultation
Our first consultation allows us to become familiar with each client and their support network. This meeting also allows for the client/family become familiar with us; both of these steps are necessary in deciding what kind of support is needed while transitioning to the Self-Determination Program.
Person Centered Meeting
At this meeting the team focuses on the individual’s aspirations and goals (current and future), as well as identifying immediate and long term needs. This meeting will allow us, the facilitators, to collect data for creating a written action plan.
Creating the Person-Centered Plan
When developing a Person-Centered Plan we gather insights and information gleaned from the consultation and Person Centered meetings. This process results in a written document that outlines the individual’s aspirations, leisure activities, social connections and future hopes. It also details the necessary supports and action plans to make these goals a reality.
At this stage of the IPP meeting, the Independent Facilitator introduces the Person-Centered Plan, outlining goals and required services to support it. Participants examine the last year’s payment records and deliberate on possible new and/or unmet needs that should be considered in the budget.
IPP Meeting
The budget reflects the participant’s total service spending over the past year. Adjustments may be needed based on changes in circumstances, resources, or if there are needs that remain unaddressed. The independent facilitator will assess the proposed budget, ensuring it aligns with the client’s needs, and address disagreements, if any, in order to work towards a common consensus.

Creating Spending Plan
The spending plan outlines the services and supports necessary to reach the IPP and Person-Centered Plan goals. It ensures the participant’s health and safety as well as details the cost of each service, activity or item funded through the SDP, including FMS, IF and employee-related expenses. This plan is then linked to the IPP.
Continued Independent Facilitator Support
Ongoing Independent facilitation services can include managing expenses and working with FMS, revising and submitting spending plans, advocating, finding services and resources, coordination with support teams, helping to achieve Person-Centerd plan goals and getting ready for the net year’s process.
